Homeowner Authorization Form

This form is used If you are a homeowner and wish for a tenant to start service at your property.

New Service Request

This form is used when a customer is requesting a new residential or commercial water or sewer service account for a property that is already connected to the Authority’s facilities.

Disconnect Service Request

This form is used when a customer is requesting the discontinuance of an existing residential or commercial water or sewer service account for a property that is already connected to the Authority’s facilities.

Leak Adjustment Request Form

This form is used when a customer is requesting a courtesy adjustment of their water bill when an undetected leak occurring between the customer's side of the meter and the outside of the house causes unusually high water usage.

ACH Billing Instructions and Form

We are delighted to introduce the Authority's Auto Pay Option for Water/Sewer bills.  With this option, your water/sewer bill can be automatically deducted from your checking or savings account.  You can now enjoy the convenience of paying your utility bill without writing checks or buying stamps.  Click on the link above for information regarding this payment option.
